Patient Forms, Map and Directions to Martinsburg Office
Your first visit to our office will consist of a full mouth set of
x-rays, dental exam and medical history review. The doctor
can then determine the right treatment to restore your smile.
Please bring your drivers license, insurance card, and list
of prescription medications. If you had bitewing x-rays
in the last 12 months, a full mouth series of x-rays, or
panoramic x-ray in the last three years please bring a copy with
you to your appointment. If the office has the ability to
email the x-rays please have your previous dentist send them.
It is the patient’s responsibility to have their records
transferred to our office.

Dr. Hartman is devoted to restoring your teeth with conservative,
state-of-the-art procedures while tailoring treatment to the
individual. We also understand that a dental visit can make a
patient uneasy, which is why we strive to make each patient as
comfortable as possible.

Driving Directions
From I-81 take the King Street exit 13 and turn toward town.
If driving north on I-81, you will turn right off the exit
ramp. If driving south on I-81, you will turn left off the
exit ramp. Go approximately 1 mile and turn left on Tennessee
Ave. You will see Eddies Tire at the corner of Tennessee Ave.
and King St. where you should turn left. Our office is located
in a white building immediately on the left side. You may park
on both sides of the street in front or behind the white
building. This part of Tennessee Ave. is one way.
If coming from King Street you turn right onto Tennessee Ave. at Eddies Tire.
